Please enter all information in full; our office needs the information requested in order to provide identifiers to your insurance that allow us to check on your insurance coverage and benefits. This is to determine the least expensive method of sessions for you. Once we receive your information, we will contact you to discuss your benefits and coverage, and schedule a consultation and appointment. Clients are required to have a credit card on file with us prior to the first session using our secure, encrypted, HIPAA-compliant system SimplePractice. Credit cards will be charged for any copay, co-insurance, balance billing, or late cancel/no show fee after each session. It is fine for someone else to place their credit card on file for you.

If you do not have insurance, the private pay rate is only $40 per 30 minute session. Payment for PRIVATE PAY sessions are between you and the instructor directly. Direct payment must be made via cash, check, or Ivy Pay in order to remain HIPAA compliant. You may also pay via Stripe using our encrypted, HIPAA compliant SimplePractice portal. If you cannot afford the private pay rate, Our nonprofit partner, Gender Voice Center, may be able to cover the cost for you. Please email info@gendervoiceslp.com to see what options are available.
